# Configure a form on an action

Forms are defined on the **action**, once: you decide the template that will be used every time that action is executed. See [Forms](/forms/) for the difference between configuring and filling in.

The action is in the **office** menu, under **Actions**, in the submenu of its type.

1. Open the action detail and go to the **Linked forms** tab. Here you see the forms already active, and you can add or remove them.
2. Click **New**. A guided wizard opens.

![The wizard that creates a form](/_astro/related-forms-1.BmNjq64-_DtRLH.webp) 3. In the first step, enter the **name** — clear and descriptive, it is how the form is identified in the system. 4. Choose the **type**. Which types are available depends on the action: see [Form types and available fields](/forms/form-types-and-fields). 5. Save, then define the content of the form. What happens next depends on the type you chose:

- [Simple checklist](/forms/simple-checklist)
- [Advanced checklist](/forms/advanced-checklist)
- [Course attendance register](/forms/attendance-register)
- [Predefined templates](/forms/predefined-templates)

From now on the form is available every time the action is executed — during an inspection, a maintenance job, a delivery — and is prompted automatically. You never have to look for it.
